United Concordia has one of the strongest dental provider networks in New York State.

• No waiting periods on Basic and Major services.

• No Pre-Existing Condition limitations

• Access to one of the largest dental provider networks in New York State

• Two (2) and Four (4) Tier pricing options

• Participation Requirements: 100% participation is required for groups of 2-4 Employees and 75% participation is required on groups of 5+ employees (spousal and other coverage waivers count towards eligibility percentages)

• Four (4) Products are available covering In-network Preventative/Basic/Major at:

HIGH OPTION            100%/90%/60% and $1500 Annual Max.

MEDIUM OPTION      100%/80%/50% and $1250 Annual Max.

LOW OPTION             100%/50%/50% and $1000 Annual Max.

BASIC OPTION         100%/50%/0% and $750 Annual Max. with
BASIC OPTION         reduced Out-Of-Network coverage.
